Il Fiat 238 fu la naturale evoluzione del 1100 T, anche in fatto di ambulanze. Arrivò nel 1967, basato sulla meccanica a trazione anteriore dell'Autobianchi Primula e la Casa madre si preoccupò di offrirne una sua versione sanitaria ufficiale, realizzata nuovamente dalla Carrozzeria Savio. In un primo tempo, la configurazione interna non variò rispetto a quella delle ambulanze precedenti, restando molto semplice, priva di attrezzature e con solo una o due barelle, un armadietto, due strapuntini ed eventualmente un piccolo lavandino. Curiosamente, all'esterno fu mantenuto il vecchio "occhio di Polifemo", ormai non più obbligatorio da quasi un decennio.

The Fiat 238 was the natural evolution of the 1100 T, in ambulance form as well. It appeared in 1967 with its front wheel drive engineering borrowed from the Autobianchi Primula car. Fiat decided to manufacture its own "official" ambulance version together with Savio Coachworks. The interior arrangements of early examples differed very little from preceding models with a simple layout of one or two stretcher carriers, a small cupboard, two fold down seats, sometimes a little hand basin but no medical equipment. Curiously, the "Cyclops' eye" lamp remained on the roof although it had not been legally required for nearly ten years.
